What Is the CHIP?

The CHIP is our shared blueprint for making the Lakes Region a healthier, safer, and more resilient place for everyone.

Developed through collaboration with dozens of local partners and updated regularly, the CHIP turns data, discovery, and lived experience into clear goals, priority areas, and measurable strategies for real improvement.

But this isn’t just a local idea, CHIP is a nationally recognized model used by communities across the country to bring people together, solve real problems, and build stronger, more connected communities.

This is not a report that sits on a shelf, it’s a living guide that aligns the work of towns, agencies, healthcare systems, schools, and organizations across the region to drive progress that matters.

Where It Begins – Our Needs Assessment

Every three years, we conduct a Community Health Needs Assessment (CHNA) to better understand the most pressing health issues in our region. This assessment informs the development of CHIP priorities and action plans.

How CHIP Drives Regional Strategy

Every PPH program and pillar is shaped by the CHIP. Our councils and committees, including Emergency Preparedness, Prevention, Human Services, and Community Health, use CHIP to align their work and evaluate impact.

 It guides grant strategies
It helps prioritize limited resources
It provides accountability and clear next steps
It connects all of us working toward shared public health goals

CHIP is led by the Winnipesaukee Public Health Council (WPHC) and supported by dozens of partner organizations across the region including:
