Your Local Aging & Disability Resource Center (ADRC)

We proudly partner with the New Hampshire Department of Health and Human Services (DHHS) to bring you Aging & Disability Resource Center (ADRC) services, your “No Wrong Door” entry point for long-term support and services.

Through strong partnerships with DHHS and community providers, we offer person-centered options counseling to help you explore choices, set goals, and make informed decisions about your care and quality of life.

Through our collaboration with local healthcare providers, Winni Mobile Health offers paramedic-led, community health worker support for Laconia residents managing chronic diseases. This program is referral-based through primary care providers (PCPs), ensuring patients receive personalized care coordination and resource navigation where it’s needed most.
